View medical records

Parents and legal guardians:

To access medical records beyond what’s available on the patient portal, you have two options:​

• Bring a signed authorization form to our office. Other healthcare providers can also request your child’s medical records by faxing an authorization form to us.​

• Make a request through MyChart for full medical charts or additional documentation.​ Most requests are processed within 5–10 business days, though older records may take more time.

Third-party requestors:

• All third-party requestors must submit an Authorization for Release of Protected Health Information (available in English, Spanish). For patients under the age of 18, this form includes the signature of a parent or legal representative. In some cases, additional documentation may be required.

Basic records


Basic records like visit notes, lab results, immunizations, and school/physical forms are available through the patient portal and may also be available for pickup at our office.

Entire chart


Requests for additional records or your child’s full medical chart must go through MyChart. For fee estimates and other details, please contact our office directly. Fees and processing time may vary depending on the request type and delivery method (digital vs. mailed).
